One Wight Health’s team of Social Prescribers work across Isle of Wight Primary Care – with patients able to access the service via their GP practice or by referring themselves.<\/p>\n
Our social prescribers also support people with cancer care reviews once they have been newly diagnosed with cancer.<\/p>\n
The reviews help ensure that there is a link between patients and their GP practice and that a holistic approach to their cancer care, rather than purely a medical approach can be undertaken. The team have undergone specific training with the support of Wessex Cancer Alliance to help them carry out supportive conversations and to be able to offer non-medical advice and signposting. This has been trialled in the North East and Central and West GP medical practices and we are looking to embed this in primary care across the Island.<\/p>\n
